OakLeaf Surgical Hospital staff understands that you may have questions regarding your health insurance plan and the particular procedures being recommended by your physician and surgeon. Our friendly knowledgeable staff is available to assist in guiding you through the insurance process and help you in whatever way possible to assure that your health insurance coverage is the last thing you have to worry about on the day of your surgery. If you have questions regarding OLSH and your insurance plan, please call 715.858.3726 or 800.635.6197 and ask to speak to our billing specialist. We recommend that you verify with your physician whether or not they participate with the following health plans when scheduling your procedure. Below is the current list (as of 2/18/08) of participating health plans and networks at OLSH. Please note, in addition to this list we will file claims to any insurance. Coverage and/or benefit level may vary based on participation status.
