What is PFAC?

The Patient & Family Advisory Council is a group of patients, family members, and hospital staff who work together to improve the quality, safety, and experience of care at OakLeaf Surgical Hospital.

Why Join PFAC?

• Share your experiences as a patient or family member.
• Help us understand what went well and where we can do better.
• Partner with hospital staff to make a difference.
• Contribute to projects that improve patient care and education.

Who Can Join?

• Anyone who has received care or had a family member receive care at OakLeaf Surgical Hospital within the last 3 years.
• No special qualifications are needed, just your experience and perspective.

What Do Members Do?

• Share stories and ideas to improve care.
• Review and help create patient education materials.
• Provide input on hospital practices and initiatives.
• Participate in council meetings quarterly.
